Managing a long-term condition takes regular monitoring, a periodic look at the medicines you are taking and a health professional who knows your history.
Your nurse practitioner reviews your current plan, requests pathology to keep track of how things are going, and coordinates with your GP and specialists. We stock home-monitoring equipment, including blood pressure monitors and blood glucose meters, in store and online.

How to book
Book an initial consultation through HotDoc. Your nurse practitioner reviews your medical history, current medicines and recent results, then works out a management plan and a monitoring schedule with you. Initial consultations typically run for 20 to 30 minutes and follow-ups for 15 to 20 minutes.

Cost & PBS
Consultation fees are confirmed when you book, before your appointment goes ahead. Your nurse practitioner will tell you whether any Medicare or PBS benefit applies to your consultation or to your care plan. Any home-monitoring equipment is charged separately; our pharmacy team can confirm the cost before you order.
